Sign for a participant under 18.

A parent or legal guardian must complete this waiver for any participant under 18. Takes about 3 minutes.

Adult participant? Use the Adult Waiver instead — anyone 18 or older signs their own waiver.

RELEASE & PARENTAL CONSENT

Prior to signing the waiver, please read the list below carefully and confirm that your child does not have any of the following. If the child has ANY of the following, please speak to a staff member.

· Uncontrolled high blood pressure or heart condition

· History of stroke, blood clots, or deep vein thrombosis

· Raynaud's disease or cold urticaria

· Peripheral artery disease or poor circulation

· Seizure disorders or epilepsy

· Recent surgery, open wounds, or active infection

· Pregnancy — known or suspected

· Severe anemia or bleeding disorders

· Uncontrolled asthma or cold-triggered respiratory conditions

· Any condition for which a doctor has advised against cold exposure
